old form being filled out

Hello.

Somehow we have an old form still being completed somewhere. The form  has been updated some time ago and somewhere, somehow, people have access to the old version (old fields and old submission instructions).

How can I figure out where that form is located?

The current (and only that I'm aware of) form is published here...

I'm checking the form in that website page but there's no notification setup on it? There's only an autoresponder on it.

Perhaps you can share us a screenshot of the email so that we can check if it matches anything in our mail logs? Also, which of your forms you are still receiving submissions? You can manually disable it by following this guide: http://www.jotform.com/help/180-How-to-Enable-Disable-a-Form
