I want my users to be able to upload an image from their phone into the form, how can I do this. I want to create a field for example suspect and then there must be a photo of the suspect next to the text. Also we want to add several evidence tabs with photo’s of the evidence next to it.
I have inspected your form http://www.myjotform.com/form/60515011007538 and I saw that you have two file upload fields. Your users will also be able to upload images from mobile devices. They will need to open gallery on their phones and select image that they want to upload.
If you want to prevent your users to submit the form without uploading image to your form, then you need to make upload field to be required.
If you want to allow your users to add text for uploaded photos, then I would suggest you to add text area fields to your from.
