I don't mean to be a bother but I created a form that needs the details from that form emailed to a subscription email. I have already set up the recipient email address to my email address to test if the details are emailed but there was no email. It did however received the details here in my JotForm account but not in my email address.
I have also set the Sender name and email address to JotForm (as seen from the other solutions) and the recipient email address to mine.
And can I also asked how to remove the scrollbars of this form embedded in a Facebook app?
Or maybe I just missed something, please let me know as soon as possible :)

With regards to your Scrollbars, is there anyway you can increase the height of the iframe container for your form? It's mostly found on your facebook addons where you've inserted the codes. Please check it there and increase the iframe height, that should fixed it.

That was surprisingly quick!
Yes, I have received the form details during submission but can I request that instead of emailing it to email@example.com, can you set it to firstname.lastname@example.org? :) I have tried to set the Recipient email address not my own, and it went anyways to my email address. So with that minor tweak, I can finally cry.
I found a solution on removing the scrollbars, so no problem on that.

First, the changes I've created to your form was reseted back or maybe it wasn't completely saved because you were also logged in that time. Anyways, you can create it on your own. I can see that your current notification is created with conditions, it is always helpful if you can create a separate notification without conditions on it and then add that email address (email@example.com) under Recipient E-mail. The conditions are set so that only those who fill out the needed information can register to the page. ^___^
