How do I make labels show in my form?

They keep hiding themselves and no one has a clue what the questions/fill in boxes are for

I have inspected your form and all labels, except last field label are shown.

Were you able to resolve this issue? 

For last field of you form, you can ad label by double clicking on blank label space or inside of filed properties.
