Why has my form been disabled?

First of all, please accept my apologies for the inconvenience. According to my checks, your account has been suspended by mistake; but I fixed the issue right now. Please, check the availability of your forms from your side and let us know.

In addition, I realized that the related form called "Henry's Shop Login" has a custom URL. If the form was suspended after customization, you can't access it through that link anymore; but with its default URL, it is accessible. So, please re-add the custom URL to that form, you will see the form is going to work again with its customized URL.
