    How can I set up one form so that each agent has their own contact form?

    Hi.  I have a real estate company with 6 agents and I want to create "button" on our website with a contact form for each agent. It seems that I can use your website to create a from that will point to one email address, but how do I set it up so that each agent has their own contact form pointing to their own email address.  Does that mean I will have to pay $39 a month to accomplish this?

    Okay so what you mean is you want a one form that has a listing of forms for 6 agents that work for you and you want each of those forms to send am email to each of their own email addresses right?

    Yes this is certainly possible and no you don't need to pay to build a form like that.

    You would only need to pay if you expect to go over the monthly submission quota for the account for such unless you divide it among 6 different accounts to lessen that probability though it may be a bit tedious.

    There are two ways to do this. You can either make it from one form using conditional logic or you can have one main form with several other forms embedded on it.

    To make it simple the second option is basically described here http://www.jotform.com/help/182-How-to-Create-a-Basic-Multiple-Payment-form but if you don't need the payment options you can leave those out as the workflow is the same. 

    You would also need to set up a notification to send to each of your agents for their email. You can alternatively set up one but have conditions change this email address if you'd rather do that.
