Email notification is not showing properly

    I am now the person who receives the JotForm for a training request, but the format of it is coming out completely different than when my boss received it (see screen shots). Could you please help me fix this?

    It is possible that the form fields were edited but the notification was not updated. Actually, the notification will not be updated automatically. It means that when you update or added a new fields on the form, you also need to update the notification.

    I would recommend to delete the existing notification and then create a new one so that the form field IDs and the table will be generated automatically just like what you can see on the screenshot you provided.
