Hello,
I'm helping a client get rid of paper consent forms. I duplicated one of their forms here https://form.jotform.com/60545231636149 My question is, I can see the submissions, but they are out of context and don't include what they actually signed. Is there a way to see the actual form they signed?
I'm just worried on the legal ramifications that you can see their e-signature, but you can see what they signed?
What's the point of e-signature if you can't show what they signed?
Any ideas?

Hi,
You can find the form with data in it by going to the submissions by following this guide.
If you would like to see that text of the form when they submit that's the way, just log in to your form builder and click submissions.
Now, if you thought to send this text to your customers when they submit, you could copy this text inside of the notifier body mail.
Just go to your email:
And than create new field and just copy the text inside of the body of the mail:
That way, your text of the agreement will go with each submission to your customers.

Thanks but I think you misunderstood my question.
If you look at the form, I have included a disclaimer and it requires their
esignature. How can you include the disclaimer also in the submission so
it shows what they signed?
Right now it shows a signature, but it doesn’t show the reason they
signed. It’s like signing a contract, but then cutting out the signature
and throwing the rest away. No way to prove what they signed. Right?

Hello,
Well, that's what I had in mind, so you want when they sign and submit, you receive all that on your email, with signature and disclaimer and all data.
To do that, copy your disclaimer text and follow this instruction:
So you can go to your email notifier and edit it, than just add new field and copy your disclaimer text there. So every time they submit, all that information together with disclaimer and signature will be submitted.
I hope that that is what you had in mind.

there a way to get it into the submissions recording somehow. Maybe a

The E-Signature is already added on the record of each submission, I just did a test and it is working on my end.
If the E-Signature widget is in your form then it will appear in the submissions, it will appear on the same order as it is in the form.
