2-We ran a test document to submit online and it went through but generated and email back to us as follows:
From: JotForm <firstname.lastname@example.org>
Subject: JotForm SMTP account error
Date: March 1, 2016 at 2:50:11 PM MST
There was an error on your SMTP configuration. Due to that error, our system automatically changed your email sender's address for your form ID 42506331335143 to email@example.com to prevent issues arising from your account.
Please log in to your JotForm account (http://www.jotform.com) to correct your SMTP configuration.
All the best,
P.S. This is an auto-generated email, please do not reply here.

I would suggest you to first delete the current SMTP settings that you have in your form, this guide will help you to achieve it: http://www.jotform.com/help/241-How-to-Edit-Delete-Sender-Emails-on-SMTP-Settings.
Once you have deleted the settings add them once again, follow the steps on this guide: http://www.jotform.com/help/238-How-to-Add-a-Custom-Sender-Address-to-an-Email-Alert.
If the issue still persists you may consider using a workaround, here is the link to do it: http://www.jotform.com/help/236-How-to-use-Mandrill-to-Send-Emails-From-Your-Own-Email-Address.
