I see that this is about form 32356444422955, which is integrated with Authorize.net payment processor. I have cloned it into my account for testing, and I too experienced some issues submitting the cloned form:
I have forwarded this thread to our developers as a bug report to see what may be going wrong with the Authorize.net payment integration.
