Please reinstate my account. I am the web designer for The Church of God's Faithful at setapartbytruth.org. I am working on the new website using MUSE and am having trouble making a passcode page. If you look at the members link you will see that we have one set up on that page. I have not used Jotform before the work on this website.
I saw that you do not use the "password" form element any more because of the phishing issue but the forum gave a work around by using a text box and conditions. That was what I was trying to do and having trouble with.
Please let me know what I need to do to reinstate my account and not have it shut down in the future.

I have checked your account/username sunrayg and I found the form that causes the issue.
