form disabled

I built several forms yesterday, went to the site today and the forms are not on the page and when I repost it says forms disabled????

One final question, the emailed form is very narrow, but these forms have long questions, say taking up 5 or 6 lines, when the answer is just YES. Is there a way to widen the delivery form? these are permanent records kept on file
