adding child records

I would like to create a form that will have child records.  (like a master / detail form)

For example, in my first try I am creating an accident  report form.  When I get to the point of adding witnesses, I can have any number of witnesses and for each one I needto have a name, phone number and address.  The user would fill in the name, address and phone of a witness and click "add another witness" to be presented with the ability to add another one or click "continue" to continue on with the master form.  In the database world, there would be 2 tables involved and a 1 to many relationship.  Can jotform handle this?  If so, I would happily upgrade to the paid level.

    Thank you for contacting us.

It is not possible to generate the witness fields automatically, but we have a workaround. Please take a look at the following thread to get additional information on this.

Can I create a dynamic response to a field

You can also redirect your customers to another form after form submission.

However, you can not manage our database to link the submissions. Could you please clarify your database inquiry in detail? Then, we will see if we can provide any solution.
