I am not sure I have understood your issue correctly. Do you mean to say that you are not able to submit form?
I did check your form and did not find any issue with that. I was able to submit your form successfully. Please check the screenshot below:
When I tried to submit the form again, but it seems to work as expected. I would suggest you to please test your form again and get back to us if you have any trouble.
