When users submit either of these two forms:
Waldorf Astoria Photo and Image Upload/Request, Conrad Photo and Image Upload/Request
They get the following error message, although I still receive a notification and the submission goes through to my inbox.
Insider - Error We're sorry, an error has occurred.
The error has been logged for analysis.
We continually monitor our error logs in an attempt to prevent ongoing problems. However, if you continue to experience problems, please contact the support desk.

This is happening on the website where you are redirecting to your submitter, I checked it and seems like it is asking for a log in.
Try testing setting the default thank you page, you will see that it is working, and please, check the link to the website where you are redirecting to your submitter, this is because you are receiving the submission, because the form is working properly.
