Fonts of my form have changed. How to resolve it?

I was attempting to change the design of the form to make the fonts larger as it is teeny-tiny and hard to read when you look at it online, however, in my attempts to change things I think I messed something up!  Can you take a look and let me know how to correct it asap?

I'm sorry for the trouble caused to you.
