Hello,
When I open the form through JotForm it is easily readable (font size/label size), however, when I open the form through our KCHDA website it is TEENY TINY form size/font size. Why is it doing this? Please help!!

Could you please provide us the link to your website, so we will take a look and see if there is something affecting the style on your form.

I'm checking the website, I see the form is not embedded on the page but is anchored to a text as a clickable link.
I was redirected to this form link when I clicked the entry form: http://form.jotform.com/60284108035246.
This is how it looks like in my end:
If you need to adjust some font size and the width of the form, you can use the Form Designer Tool. Here's a guide to learn more about it: http://www.jotform.com/help/300-Form-Designer-Tutorial-Let-s-create-fantastic-forms-
If you need to embed the form on your website, you can follow this basic guide that we have: http://www.jotform.com/help/34-Adding-a-Form-to-a-Web-Page. I do recommend that you used the iFrame embed code to make sure no script conflicts will happen.
