It appears that your account was auto-suspended by our phishing detector. Our phishing detector checks if a form has fields that ask for a user's sensitive information such as passwords and credit card information. These type of fields are a violation of Jotform's terms.
I checked your account and it appears that your form does not violate our terms, so I have decided to lift the suspension on your account. You should now be able to view your form without any issue.

Hello @Christian. Why does no one ever answer private submissions? I have had multiple message go unanswered. Some have been urgent. Please assist, and contact me via e-mail.
