Hello, I did an upgrade on march 20th (see annex). Today (21/03/2016) I received a mail telling me that my forms were disabled. Can you please resolve this. Thank you.

Unfortunately, we had some issue with upgrading accounts when subscribed using PayPal which is now resolved. Hence, your account was not automatically upgraded. I've manually upgraded your account now to Bronze and you should be able to see the forms are activated.
