JotForm does not have form workflow feature but you can create this three levels of approval using conditions and Email Notification features.

Basically to have those three levels of approval what you would do is create the form normally will all the needed fields and add three hidden section in the form for each level (Admin to approve the from or request ). Then add a PassCode field on top of the form that will show an admin section depending of the code passed into it.

You will need to create an Email Notification for each level and add the PassCode in it so whenever an admin receive the email with their PassCode so they can open the form and approve it or not.

If you need to notify the person filling the form you can create an Autoresponder Notification and use conditional logic to send an approved email if all the admin approved the request or otherwise.

I have created a DEMO you can clone to see the internal setup: Request - Approval

To test it 'Select Admin Level' - the PassCode are respectively:

    admin1

    admin2

    admin3
