    Hi,

    My domain name is: uniyearbook.com

I followed your instruction, and my domain provider changed my DNS details as it was explained here:

    http://www.jotform.com/help/144-How-to-use-Custom-Domains-with-Jotform

I choose to call my forms: registration

registration.uniyearbook.com it points to Jotform now. But how can I manage to rename my forms at Jotform?

I would like to call them: registration.uniyearbook.com/lse  but where can i set this up in Jotform?

I enclosed the picture where i tried to set this up, but it still do not let me change the first part of the domain: https://form.jotformeu.com/60814870308355

    Hi there,

The good news is you have already done it properly; but the URL on the "Publish" wizard won't change automatically, you need do this manually after applying the necessary steps. Below, you will see the working link for your form " LSE 2015/16 Yearbook Registration":

    http://registration.uniyearbook.com/Uniyearbook/ucl-yearbook-2015-16

Please, follow the last part of the related guide:

Change your form URLs from http://www.jotform.com/form/1234567890 tohttp://forms.my-company-domain.com/form/1234567890

Please, check it on your side and let us know the results.

    Regards.

Thanks for your reply. I see. One more question:
After submitting form the link will be Jotform again, can we also customize this? or is it not possible?

You are most welcome. If we come to your next question, you can also beat that problem in theory, By changing the "Thank You" page with the one page from your domain, the submit page's link will be changed with one of yours.

Please, check the guide from the below:

    http://www.jotform.com/help/211-Setting-Up-the-Thank-You-Page

I hope this one will meet your requirements. Please, do let us know the results.

    Thanks.
