my form is disable and it is so annoying

You account was suspended for creating log in forms.  We do not allow for forms that request usernames or passwords for any reason.  Since nearly all of your forms ask for such information, your account and forms will remain locked.  If you choose to create new forms, please refrain from asking for any sensitive data (usernames, passwords, credit card details etc) as it will likely result in your account being suspended once again.
