I created forms and in the email notifications section I checked the box to attach as pdf to the email. Some forms when emailed contain all the text from the form on a pdf but other forms only show the field boxes and entries on the attached pdf but not the "text" only sections. The example is my Family Law Telephone Consultation Follow up - Morgan Law Firm form.

I think i got it figured out. Submissions page, settings, check box show headers and text.

Glad you were able to figure it out. As you mentioned, you can enable the TEXT and header fields to be displayed from the submission page, under the setting options.
