Hello adykhouse,
Yes it is possible to achieve your requirement. On click on a radio button option, you cannot redirect user to another form but it is possible to display another form within the same form. Here are the steps you should follow:
#1. Create a master form where you want to display the radio button question.
#2. Create a second form which you want to display when radio button option is selected.
#3. Grab the form's direct URL of the second form. Please check the screenshot below how to grab your form's direct URL:
#4. Add "Iframe Embed" widget in master form and add the form URL which we copied in step #3.
#5. Add a condition in your form to show / hide the iFrame based on the selection of radio button option.
