Problems with calculating percentages

My from "clone of Orientation Form" is still not working. All I want is the calculations to work.

I have been trying for days. I have watched every video and i can not get the score or percentage from the calculation wizard to work.

I checked your form and it looks as though the correct values are shown.  The calculation values associated with your answers are summed up in the score field and then the percentage is calculated in the percent field.  When I tested the form, a score of 5 was shown for my test entries and a percentage of 29.41 was shown in the calculation.  (5/17)*100 is indeed 29.41.  Which part of your form is incorrect?
