My forms say "Form not found. This form is disabled."

I embedded the iframe code onto my website, but when I go to view it, it says "Form not found. This form is disabled."

Please advise ASAP. This is how clients contact us.

Your account was suspended automatically by our phishing detector tool. It is because the form is asking for credit card information. This is a violation of our terms of use. Here's a screenshot:

You need to use our payment processors in order to charge the customer via credit card.

We can re-activate your account but we need to delete that form. Let us know what you think.
