Form displaying as "Form Disabled"

    I recently set up a pretty data-heavy form; a series of self-filtering drop downs with more than 5000 options total. I'm trying to port into SquareSpace, but when I copy past the code I get the image attached. Am I overloading JotForm, or trying to accomplish something beyond the price range of a free account? What's going wrong? Overall I've been extremely impressed with JotForm's capabilities, I'd hate for this to get in the way of an otherwise excellent experience.

    I would like to apologize for any inconvenience. However, I checked the form now with its URL and it is not disabled already.

    The form might have been suspected of collecting sensitive information by our Anti-phishing system and marked it. 

    You can check the form now as it now working. 

    By the way, the form belongs to a guest account. If you want to have it saved to a personal account(free account) create an account using the same email address you have used when you created the form.
