Your form was suspended due to phishing, this happens when a form is asking for information that is not allowed by JotForm, however, I checked you form and seems to be clear, it could be because your form is not in English, please take a look on this guide and set English as the default language and then add your language: http://www.jotform.com/help/298-How-to-translate-your-forms-to-different-languages.
I have restored your account and you should be able to load your form fine now.
