Configurable list widget: Checkbox input not being submitted

I'm having trouble with this form

When a checkbox is checked, it does not register on the submission. 

See attached PDF. All the checkboxes were checked but this does not register in the submission. 

That's because you have an empty option, in other words, you have not added an option, example:

In a check box, the option or value(can be a number, letter, word etc) selected will be displayed. So, you might add a "yes" option, or "yes" and "no" options.
