I need to reset my password to my account urgently and am not recieving the reset instructions via email.

As of checking, the form you're referring to is under a guest account. You may open JotForm with the last browser that you used since guest accounts are retrievable via cookies of your browser. If it's no longer possible, you will need to create your account by clicking the Sign-up button on the top-right side of the JotForm page and use the same email address that you have used.
If you continue to experience problems with accessing your form after the creation of your account, please do let us know and we'll have the form moved to your new account.
