We have a large group of people using the form that we purchased and keep getting failed alerts when trying to upload, even .zip files.
If someone could give me a call ASAP that would be great,

As for your concern, I cloned your most recent form and tried to upload a zip file. I was able to upload the file successfully.
It appears that the file from your screenshot that's being uploaded has an invalid extension. The file is named Blankslide4_Hispani...Charlotte.mov.zip. Since the file ended in .mov, the file upload might have confused it as the extension of the file instead of .zip. To fix the issue, I would suggest you remove the .mov and changing the name of the file to Blankslide4_Hispani...Charlotte.zip.
Alternatively you can also set the upload field to directly accept the .mov extension so you can upload .mov files. Here's a guide that can help: Upload Field Allowed File Types.
