I am not sure but I cannot preview my form or anything. It is disabled. Am I doing something wrong?

Your account was autosuspended because one of your forms is asking for a computer or email password. You are also asking for users if they have an Apple ID and if they know the password.
I see you seem to be using the form for IT assistance, I have removed the guilty fields and re-activated your account. Please do avoid creating similar form fields, please check our terms page for the things that are not allowed in the form creation.
