Excel Format

    Thank you for the reply about my form.  I think I'm on the right track.  However I'm including a screenshot for more clarification.

    After I get submissions and I download the csv or excel file with the information I want the column titles to be in particular format so when I upload this information into my database the required fields are already correct.  I see where to change the title, however how do I do that on a "Name" that has one place but 3 different columns.  First, middle, and last name.  I need each column to read differently.  Take a look at the screenshot.  That will give you an idea of how I need the output to be.  I added my needed format on the line below and highlighted it in yellow.

    If you need to change the labels for default fields, you would need to use text boxes instead.  Rather than using a Full Name field, create 3 text boxes and label them first_name, middle_name and last_name to match your requirements.
