PayPal/Check option payment form

    The second issue is that the fee schedule allows participants to select what they are signing up for and even if paying by check, they need to see both the field, "fee schedule" and the total, even if just in the email that gets sent to them.  In our spreadsheet that Jotform creates, we are also missing the payment amount information, so we don't know what they owe either.

    If I understood you correctly you want to allow your users to select product on your form and once the products are selected if users select  Check option, product that they have selected should be shown on submission page of your form and inside of email that is sent to your users.

    I have cloned and tested your form and currently products that are selected inside of PayPal payment field will not be shown inside of submissions, if Check option is selected. This is happening because of the condition you have set inside of your form to hide Paypal field if Check option is selected.

    Workaround for this issue is to create list of you product fields outside of payment field. Then, same as now you will need to have payment option field. If the check option is selected your users will be able to submit the form and all submitted data will be shown on your submission page. If they select PayPal, Paypal payment filed will be shown and total amount will be passed to PayPal field and once your form is submitted your users will be redirected to PayPal.
