Need help in creating a form

Could I send a copy of this form in a word doc to somebody so they can assist?

The message you posted doesn't appear to have any information about the form or details regarding what you need to achieve with it. 

I see that you mentioned above that you need to send a copy of a form in a Word document - Actually if you want to share a form with someone using offline document, I will suggest using  Fillable PDF instead as it is more convenient. This feature is also accessible under forms options.

Yes. Fillable PDF forms collect data and can store the data to your JotForm account when submitted while the user is connected to the Internet. If the user is not connected to the Internet, they may save the PDF file after filling the information and send the form back to you using email attachment.

You may create the fillable PDF form once you are done with creating the online version of your JotForm. Please let us know the form ID/URL that you are seeking assistance and more information on your requirement so that we can better assist you.
