When Customers submit our wholesale form, it just goes to a blank screen!

Customers have been telling us once they submit our wholesale form it just goes to a blank screen, So many customer send us 3 or 4 because they are uncertain it went through!  Ive added the thank you and even previewed it and it seem ok, but, when I submit from my site it goes to a blank screen?  Please advise on how I can fix this!

The form embedded in your page here http://www.goddessofspring.com/wholesale/ is sometimes not working due to script conflict. To fix it, please re-embed your form using an iframe embed code. This will prevent any conflicts on your page since the form will load in an iframe container which serves as the form shield.
