When you share a form as as a template do your email notifications get removed?

We are sorry for the inconvenience this may caused you. I've tested sharing form as a template. I am not able to reproduce the same issue that you're having. My notification was still there. I've checked your notification on your form which is http://www.jotform.us/form/60966970139164. It seems there are missing fields on it.
You can solve that issue by adding a Recipient Email.
