Account suspended by Phishing Detector for asking Facebook details

Hi! I am having trouble with my form. For some reason, it has been disabled. I looked into this and believe it is because I have the work 'FACEBOOK' on my form.

Is there anyway to reactivate this? Thanks!

We do not allow collection of login credentials, as described in our terms, which is why asking for Facebook username will rank very high by our Phishing Detector tool. Same goes for Google+, Youtube, and others.

As your form does not seems to be used for phishing, and other forms in your account are also fine, I have re-enabled your account. Your forms should be working again.
