Form not found - account is autosuspended

Do I have to start over? How do I retrieve this form? And how do I send it to the recipient. Can I post this form as a link on my Facebook page or website. Please advise.

Your account is currently suspended. Your form is considered as a phishing form. It is asking form credit card details and online banking login credentials. This is not allowed in JotForm. It also violates our terms of use. 

We can re-activate your account but we need to delete the forms. Thank you.
