    Sure adding background image is easy with the use of  Injecting Custom CSS to do this:

    1. While  on  your form (Edit Mode), click Setup & Embed then  Preferences

    2. On the Preferences window, goto Form Styles tab and then insert the following CSS Code under Inject Custom CSS Box. Make sure that the url for the image is correct and working.

    .form-all
    {  backgound-image:url('http://www.site.com/image.jpg'); }

    You might need to add this just in case:

    .form-line { background: none; }

    Thanks it worked using the css code from the link you sent me. I tried the first one and it didn't work. I still wish I my writing was slightly transparent so I could see the background slightly. I thank you very much.

    Thanks now I got the spread options working now how can I be able to pick multiple answers from the radio buttons or checkboxes? I have to shorten my form and this will work great but I have to be able to pick more than one answer per question. Thanks so much.

    To enable multiple answer per question I would suggest using the Checkbox field from the form tools, this allows the user to select more than 1 answer to a question. The radio button does not have the option for multiple selection unlike the checkbox.

    Thanks!

    Could you send me the code you sent the person above? The first one you posted did not work.

    @womenselevationmag

    Please inject the following css to add image to your form's background:

    .form-all {

    background: url(IMAGE_URL) no-repeat;

    }

    Please check this guide about How to Inject Custom CSS

    Thanks,
