Form not found and disabled

Hi,

The forms on my account have been disabled, I believe from the antiphishing system. How can I go about getting them reactivated? I appreciate your help! 

Your form http://www.jotform.us/form/53495456743163 was categorized by the Phishing filter as a phishing form because it captures email password

The Phishing filter in placed had already Autosuspended the forms and account.

If you will delete the password field or delete the form, we can request for the re-activation of the account.
