Why the name of my form is still no update?

As you can see the name is still CV submission but I hade challenge this one for "Formulario Enterarte" I will really thankful if you can help me I can´t do teh forma again because I have printed my visiting cards with this adress

To resolve the issue, please change the Page Title of your form into "Formulario Enterarte". Simply click the Preferences button on your form builder, select General Settings and click the "Form" beside the Page Title to change it.
