Why my form only displays the submit button even when I have added fields?

I have created a form, however when i preview it only the title and submit button load up

I have also  tried to open it on another device via the URL and the same thing happens however the entire form has saved, and loads up in form editor even after logging out/in

I cleared your forms cache and the full form should now be showing.  It seems as though there may have been an older cached version that was displaying.  If you are still unable to view your full form on your end, let us know and we will be happy to have another look.
