My acct has been suspeded due to possible phishing.
I need to know how to get the suspension lifted ASAP. My clients use these forms daily to send me card authorizations.
WHO DO I CONTACT? I have already sent a request by the link when I try to log in but no response.

I said there that this was due to three forms in your account; however, I did check all of them and found that you are asking for CC info in all your forms, I provided some links that will help you to properly collect payments and avoid phishing.
