It is telling me to upgrade my account when we already are paying 9.95 per month - are forms will be disabled it is saying - I would like to talk to some one about this. Looks as though my account has been downgraded - not sure why - please advice

The reason might be you have already reach the 1000 submission limit or space for your premium plan

I see no issues with your account.
Its at Premium status. You may have needed to log out of your JotForm account and log back in to see the change in status after your upgrade.
Can you please get in touch if you are still seeing some kind of problem.
