    It happen on the last 2 submissions.  Their email and telephone number was not on my notification form sent to me and it is not in area, where I can print a pdf or excel.  I also just received an call from a would be client that they could not enter online. So something is going on, but I don't know what.

    @patriccia,

    I have had a look at your last 2 submissions and I note they are from the same user. Its very possible that this user is using a browser which doesnt have javascript enabled.

    This may well serve as explanation of what has happened with those 2 submissions.

    The form validation requires javascript to be enabled for it to work.

    Okay, the reason you gave me, is not valid.  I received 4 more submissions and they did not have the email and telephone number on them. This never happen before.  Now I have problems of trying to get in touch with these client.

    Can you look further into this, to see what the issue is.  Please.

    It is notable that these 4 new submissions all come from the same user/PC.

    However, we can look at your website to see if any possible javascript related problems are happening there.

    Do you have a URL of that webpage, please?

    If there some kind of code conflict on your website/webpage , we will need to see the page in which the form is embedded.

    (Please advise if there is any privacy issues and we can make the thread Private in advance of that)

    Is the forms that is missing this data coming from people who already have an AKC number?

    Ok, it is possible that some script on your website is interfering with the javascript validation on the JotForm form.

    The usual fix for this is to use an iFrame to embed the form on your website, because you are you using php to generate these forms pre-filled, this is not as straightforward as it normally is.

    Do you know the line of php code responsible for generating the embed code? We are going to have to update it to generate an iFrame embed instead.

    The easiest way for us to do this is for you to post the line of php that generates the form, and we will provide a line of php which generates an iFrame, which you can use in its place.

    $phone = $row['PHONE'];

    $email = $row['EMAIL'];

    echo '<script type="text/javascript" src="https://secure.jotformpro.com/jsform/jotform#?reg='.'&phone='.$phone.'&email='.$email.'"></script>';}?>

    OK, you can try this code in its place:

    $phone = $row['PHONE'];

    $email = $row['EMAIL'];

    echo '<iframe allowtransparency="true" src="https://secure.jotformpro.com/form/jotform#?phone='.$phone.'&email='.$email.'" frameborder="0" style="width:100%; height:1534px; border:none;" scrolling="no"></iframe>'; } ?>
