Unfortunately, page breaks in the fillable PDF form is not yet possible as of the moment. You can check this guide to know more about it: https://www.jotform.com/help/312-Fillable-PDF-Everything-you-need-to-know-about-JotForm-Fillable-PDF-Creator

In that guide, I have quoted this part of it:

"Q: Will Page Break or some other field/tool create a page break in PDF for us?

A: No, not at this time. Each PDF that you create will be a single page that will not be impacted by the Page break or Form Collapse."

What the option "Include page breaks" does is that it automatically separates the form in pages based on the acceptable printable area. If you disable that option, it creates a one whole page PDF form.
