I disabled my forms

Hello community JotForm, I have disabled me my forms, and do not understand the reason that happened, I would like to know the reason not to continue retrying in creating such data, or if there was my mistake, I hope as soon as possible me enable my forms, they are greatly appreciate it, I really appreciate the work you do, so try to meet each of its regulations, but I require to know the reason why it happened.

It appears that your account was auto-suspended by our phishing detector due to this form: http://www.jotformz.com/form/61077223178657. Please note that collecting login credentials (username and password) is not allowed in jotform. This is a violation of jotform's terms. 

I would suggest deleting these fields so that the suspension on your account will be lifted.
Once you have deleted the fields, please let us know on this thread.
