    How to include a form field in the email subjet?

    When receiving form responses, I would like to encode a unique Email Subject so gmail doesn't thread each response.

    For example, I'd like to add {id4} and add it to subject text.

    How would I do this?

    Yes, it is possible to do, you only need to open the email wizard to that, as soon as you click on the email icon, on the top toolbar in the Form Builder, you will get the wizard to customize the email template. 

    Click on edit on the email notification that you want: 

    Now, you will find the form fields by clicking on the option Form Field, then once you click on the name of a form field this will be added and it will look like this {name}, you will need to copy and paste this value in the subject of your email. 

    You are free to include in the email subject the fields that you want, if you want to set a unique value for each submission you should add the Unique ID widget to your form, you will find it on this link: http://widgets.jotform.com/widget/autoincrement
