    Unfortunately, I was not able to find any account with your email address. Is it possible to give us the URL of one of your forms. This will help find the account in question.

    It looks like that you use an old version of our form builder, and there
    is currently some database connection issue. It has been reported to our
    developers.
    In the meantime, you may consider switching to our new version of the
    form builder. You have an account which was imported from old version to
    the new version. Account name is *cccyouth* and account email address is
    *youthministries@cccsda.org*.

    Thank you for the update. As indicated in the response by my colleague, you have an account with the username cccyouth. This account is associated with the email youthministries@cccsda.org.

    If you forgot your password, you may use the FORGOT PASSWORD link with the email indicated.

    An email will be sent to that email with instruction on how to reset your password.
